Victoria, Laguna, January 5, 2018 –In line with the Presidential Decree No. 1572 (P.D. 1572), the Department of Trade and Industry – Laguna Provincial Office (DTI – Laguna) is reminding the repair and service shops and centers in Laguna to submit their application for accreditation at their office located at Km. 75, National Highway, Brgy. Banca – Banca, Victoria, Laguna not later than January 31, 2018. Below are the requirements: 

1. Duly accomplished Accreditation Application Form (please find attached form)
2. Copy of Comprehensive General Liability Insurance Policy and Receipt of payment. Insurance period must be at least up to the end of the year (December 31). For on-site job, submit Affidavit of on-site job in lieu of CGLIP.
3. Warranty (please find attached Warranty Form)
4. Any changes/amendments in the following:
5. List of new Tools and Equipment & Corresponding Value
6. List of Shop Employees (name/s and inside)- 2 copies

III. Size of Shop/Stalls

1. Photo of Shop (Front and Inside)- 2 copies
2. Shop Layout
3. Organizational Chart

VII. Location Map

1. Copy of Business Name Certificate of Registration
2. TESDA Certificate of Competency for Technical employee/s for 3 to 5 STAR Category only. However, mandatory for accreditation of Refrigeration and Air-conditioning [RAC] and Mobile/Auto Air-conditioning [MAC] regardless of STAR Classification.
3. Performance Bond of P50,000 (for 3-5 STAR Category)
4. Dealership Agreement for 5 STAR Category only.
5. Latest Accreditation Certificate
6. Proof of ownership of recovery machine for Aircon and Refrigeration

Failure to comply would mean violation of the said P.D. 1572 that requires all service and repair enterprises for accreditation.

The accreditation is a form of government assurance to consumers that they can get standard services and competent repair service men attested by the government’s Technical Education and Skills Training Authority (TESDA) when getting a service from the shop.♦
